Amelia
Amelia is a previously healthy 10-year-old Black female of Jamaican descent presents with a 2-day history of “seeing double.” Symptoms have gradually worsened over the 2-day interval. No vertigo, hearing loss, focal numbness, or motor weakness is reported. She does note that she has felt fatigued for the past 3 months. On direct questioning, she feels a “lightning bolt” sensation with forward neck flexion.
Jamie
Jamie is 15-years-old and recently diagnosed with relapsing remitting multiple sclerosis (RRMS). Her first attack occurred 1 year ago, manifesting as optic neuritis. She was started on fingolimod and vitamin D supplementation. Her second MS attack occurred 3 months after initial presentation and about 4 weeks after initiation of fingolimod therapy.
Brianna
Brianna is a 16-year-old who presents with visual loss worsening over 3 days, involving the right eye initially followed by the left eye. She reports pain with extraocular movement. On examination, visual acuity is 20/200 right eye (OD) and 20/100 left eye (OS). She is unable to identify several color plates.
